Active Medication Regimen
Chemotherapy Protocol: FOLFIRI + Bevacizumab
Protocol Name: 5-FU/Leucovorin/Irinotecan (FOLFIRI) + Bevacizumab (Avastin)
Indication: Stage IV metastatic colorectal cancer with liver metastases
Cycle: 14 days (biweekly)
Components
Bevacizumab (Avastin)
- Dose: 5 mg/kg IV
- Frequency: Day 1 of each cycle
- Route: Intravenous infusion
- Mechanism: VEGF inhibitor (angiogenesis inhibitor)
Irinotecan (Camptosar)
- Dose: 180 mg/m² IV
- Frequency: Day 1 of each cycle
- Route: Intravenous infusion
- Mechanism: Topoisomerase I inhibitor
Leucovorin (Folinic Acid)
- Dose: 400 mg/m² IV
- Frequency: Day 1 of each cycle
- Route: Intravenous infusion
- Purpose: Potentiates 5-fluorouracil
5-Fluorouracil (5-FU)
- Dose: 400 mg/m² IV bolus + 2400 mg/m² continuous infusion
- Frequency: Day 1 (bolus), Days 1-2 (continuous infusion)
- Route: Intravenous
- Mechanism: Antimetabolite chemotherapy
Supportive Medications
Antiemetics
- Ondansetron (Zofran) — 5-HT3 antagonist for nausea/vomiting
- Dexamethasone — Corticosteroid for nausea prophylaxis
- Aprepitant (Emend) — NK1 antagonist for breakthrough nausea
Anti-diarrheal
- Loperamide (Imodium) — For chemotherapy-induced diarrhea
Neuropathy Management
- Gabapentin — For peripheral neuropathy
Chronic Medications (Non-chemotherapy)
Cardiovascular
- Antihypertensive agent — Blood pressure management
- Statin — Lipid management
Other
- Multivitamin with minerals — Nutritional supplementation
Monitoring Parameters
- CBC with differential — Before each chemotherapy cycle
- CMP (metabolic panel) — Before each cycle and periodically
- LFTs (liver function tests) — Regular monitoring given liver involvement
- CEA level — Tumor marker monitoring
- Imaging (CT) — Every 2-3 months for response assessment
