Active Medication Regimen

Chemotherapy Protocol: FOLFIRI + Bevacizumab

Protocol Name: 5-FU/Leucovorin/Irinotecan (FOLFIRI) + Bevacizumab (Avastin)

Indication: Stage IV metastatic colorectal cancer with liver metastases

Cycle: 14 days (biweekly)

Components

  1. Bevacizumab (Avastin)

    • Dose: 5 mg/kg IV
    • Frequency: Day 1 of each cycle
    • Route: Intravenous infusion
    • Mechanism: VEGF inhibitor (angiogenesis inhibitor)
  2. Irinotecan (Camptosar)

    • Dose: 180 mg/m² IV
    • Frequency: Day 1 of each cycle
    • Route: Intravenous infusion
    • Mechanism: Topoisomerase I inhibitor
  3. Leucovorin (Folinic Acid)

    • Dose: 400 mg/m² IV
    • Frequency: Day 1 of each cycle
    • Route: Intravenous infusion
    • Purpose: Potentiates 5-fluorouracil
  4. 5-Fluorouracil (5-FU)

    • Dose: 400 mg/m² IV bolus + 2400 mg/m² continuous infusion
    • Frequency: Day 1 (bolus), Days 1-2 (continuous infusion)
    • Route: Intravenous
    • Mechanism: Antimetabolite chemotherapy

Supportive Medications

Antiemetics

  • Ondansetron (Zofran) — 5-HT3 antagonist for nausea/vomiting
  • Dexamethasone — Corticosteroid for nausea prophylaxis
  • Aprepitant (Emend) — NK1 antagonist for breakthrough nausea

Anti-diarrheal

  • Loperamide (Imodium) — For chemotherapy-induced diarrhea

Neuropathy Management

  • Gabapentin — For peripheral neuropathy

Chronic Medications (Non-chemotherapy)

Cardiovascular

  • Antihypertensive agent — Blood pressure management
  • Statin — Lipid management

Other

  • Multivitamin with minerals — Nutritional supplementation

Monitoring Parameters

  • CBC with differential — Before each chemotherapy cycle
  • CMP (metabolic panel) — Before each cycle and periodically
  • LFTs (liver function tests) — Regular monitoring given liver involvement
  • CEA level — Tumor marker monitoring
  • Imaging (CT) — Every 2-3 months for response assessment
